Key Responsibilities:
- Daily visits to properties to undertake needs reviews, prepare outcome plans, and risk management plans.
- Make daily case notes and liaise with health professionals.
- Support clients to develop skills to live independently.
- Manage disputes, enforcement of Licence Agreements, and liaise with Police as required.
- Participate in the Reach for the Stars empowerment programme.
- Prepare properties for habitation and undertake minor maintenance where safe.
- Assist residents to obtain independent accommodation.
- Report safeguarding concerns and complete internal paperwork.
Essential Attributes:
- Experience in similar roles with clients having complex needs.
- Conflict management and de-escalation skills.
- Good communication and record-keeping skills.
- Valid driving license and access to a vehicle.
- Willingness to work flexible hours and participate in call-out rota.
